Documents required
- Relevant medical history – A full GP summary is required (free from your GP or on your NHS App). For public service vehicle (PSV) licence medicals, this is mandatory. If you're facing difficulties, contact us for assistance.
- Glasses prescription – If you wear glasses or contact lenses while driving, please bring your up-to-date eye prescription, which can be obtained from your optician.
- Medical test results – Provide any relevant results.
- Current medications – Your repeat prescription is ideal.
- Identification – Bring your passport or drivers licence, along with any additional ID required.
- Application form – Ensure you bring the correct form.

D4 DVLA Medical
£45
You can modify or cancel your booking, but we require at least 48 hours’ notice. If less than 48 hours’ notice is given, we won’t be able to offer a refund for the appointment.
DVLA PSV - D4 Form
Lorry and bus drivers are required to complete a DVLA medical using form D4 in the following cases:
- Annually from the age of 65
- When applying for a PSV licence for the first time
- At age 45, and then every five years thereafter
The medical exam takes approximately 20 minutes.
Important information - HGV driver medical
If you’ve had ischaemic heart disease (like a heart attack, angina, or a stent insertion), a cardiologist supervised treadmill test is required, though this is not part of the standard medical.
For insulin-dependent diabetics, you must see a specialist annually and monitor blood sugar twice daily.
If you take diabetes medication, daily blood sugar checks are also necessary.
For high blood pressure, provide at least three home readings taken on different days if you’re on medication.

Free re-test
At OneMedicine, we understand that unexpected factors may impact the outcome of your medical assessment.
If you do not pass, we offer a free re-assessment within 60 days of the original test. Please note that this re-test must be completed within the 60-day window, not just scheduled. After this period, the free re-assessment offer will no longer apply.
This policy ensures you have a second chance to meet the necessary health requirements without additional cost, giving you time to address any concerns.
