Transoesophageal Echocardiography (TOE)
Transoesophagel echocardiography (TOE) is an advanced imaging test used to create detailed images of your heart and the blood vessels around it. Unlike traditional echocardiograms, which use a device placed on the chest, TOE involves a small probe that is inserted into the oesophagus to provide clearer and more precise images of the heart’s structure and function. At OneMedicine, we offer TOE to help diagnose heart conditions with accuracy and guide effective treatment.
What is transoesophageal echocardiography (TOE) ?
TOE is a specialised type of echocardiogram that uses sound waves to create detailed images of the heart. The probe is gently passed into the oesophagus, which is located close to the heart, allowing for clearer images than traditional external echocardiograms. This test is particularly useful for assessing heart valve function, detecting blood clots, or identifying abnormalities in the heart chambers.
Because the probe is positioned closer to the heart, it provides high-resolution images that are not obstructed by the chest wall, lungs, or ribs, making it an invaluable tool for diagnosing a wide range of heart conditions.

Why should you consider a TOE?
TOE is particularly recommended when a standard transthoracic echocardiogram doesn’t provide enough detail or when doctors need to get a closer look at specific areas of the heart. It’s often used to:
- Evaluate the heart valves for any signs of malfunction or disease.
- Detect blood clots, particularly in patients with atrial fibrillation or prior stroke.
- Assess the aorta for aneurysms or dissections.
- Examine the heart’s chambers and blood vessels in detail.
- Guide surgical or interventional procedures related to heart health.
This test is particularly useful if your doctor needs to assess conditions like endocarditis, valve disease, congenital heart defects, or if you are being evaluated for certain surgeries.
What can TOE detect?
Transesophageal Echocardiography is highly effective at identifying a range of heart conditions, including:
- Heart valve disorders: Such as regurgitation or stenosis, where the valves do not open or close properly.
- Blood clots: Especially in the left atrium, which could lead to stroke.
- Aneurysms or dissections: In the aorta or other major blood vessels.
- Infective endocarditis: An infection of the heart valves or lining of the heart.
- Congenital heart defects: Structural abnormalities that are present at birth.
TOE provides a detailed view that helps doctors make accurate diagnoses and develop effective treatment plans.
How is a transesophageal echocardiography performed?
TOE is a more involved procedure than a standard transthoracic echocardiogram, but it is still safe and straightforward:
- Preparation: You will need to fast for several hours before the test. You may also be given a mild sedative to help you relax during the procedure.
- The Procedure: A local anaesthetic will be sprayed into your throat to numb the area and make the procedure more comfortable. A thin, flexible probe is then gently passed through the mouth and into the oesophagus. The probe emits sound waves that bounce off the heart, creating detailed images.
- Duration: The test typically takes around 30 to 60 minutes, depending on the specific images required.
- After the procedure: You’ll need a short recovery period to allow the sedation to wear off. Once you’re fully alert, you can return home, although it’s recommended to arrange for someone to drive you.
