Medical Assessment Pathway

Memory Clinic Pathway

A step-by-step approach to understanding and managing cognitive health concerns

Our Step-by-Step Approach

1

Doctor Triage & Screening

A calm, unhurried consultation to understand concerns, daily impact and goals. We review medical history and medicines, and plan the assessment.

2

Health Checks & Blood Tests

Baseline observations plus targeted tests (commonly full blood count, kidney/liver function, TSH, B12/folate, glucose/HbA1c, Vitamin D, and others if indicated). ECG or urine tests may be arranged where clinically appropriate.

3

Cognitive Profiling

We use validated tools suited to the person—such as ACE-III, MoCA, RUDAS or clock-drawing—adapted for language, culture and education. These assess memory, attention, language, visuospatial skills and executive function.

4

Brain Imaging

Where recommended, we arrange MRI (preferred) or CT via trusted local centres.

Note: Imaging fees are billed directly by the radiology provider and are not included in our clinic fees.

5

Specialist Review & Diagnosis

An experienced clinician (old age psychiatrist/neurologist partner) reviews findings alongside Dr Ahmed to provide a clear explanation, differential diagnosis and recommendations—aligned with NICE guidance.

6

Personalised Management Plan

  • Medication options where appropriate (e.g., treatments used in Alzheimer's disease)
  • Strategies for sleep, routine, activity and cognitive stimulation
  • Risk-factor management (blood pressure, diabetes, lipids)
  • Safety advice (driving/DVLA guidance, falls, medications, home setup)
  • Carer support and links to local community resources
7

Written Report & Follow-up

You'll receive a plain-English report to share with your GP, family or workplace, plus a review plan so you're not left on your own.

Cognitive & Memory Tests We Use

ACE-III

Addenbrooke's Cognitive Examination: broad assessment across five domains

MoCA

Montreal Cognitive Assessment: sensitive screen for early changes

RUDAS/GPCOG

Options for diverse backgrounds and primary-care settings

Clock-drawing & Verbal Fluency

Quick measures of planning and language

Tests support the diagnosis but are interpreted in context with history, examination and investigations.

Why Early Assessment Matters

Spotting problems early allows timely treatment, better planning, and support for independence. It also helps families understand what's happening and how best to help.

Clinic assessments and GP reviews are covered by our fees. Imaging (MRI/CT) is arranged externally and paid directly to the radiology centre.

Where clinically appropriate, yes. We’ll explain benefits and side-effects, monitor safely, and can liaise with your NHS GP if you wish.
