The infection develops in stages: primary, secondary, latent, and tertiary, each with distinct symptoms. If untreated, it can lead to serious health complications, including damage to the heart, brain, and nervous system.

Syphilis is transmitted through direct contact with a syphilis sore, which can be found on the genitals, anus, rectum, lips, or inside the mouth. It can also be passed from an infected mother to her baby during pregnancy. Syphilis cannot be spread through casual contact like sharing toilets, clothing, or utensils.

You can reduce the risk of contracting syphilis by:

  • Being in a monogamous relationship with a partner who has been tested and is syphilis-free.
  • Using condoms consistently and correctly during all forms of sexual activity to prevent contact with syphilis sores.

  • Are sexually active, particularly if you have unprotected sex.
  • Are a gay or bisexual man.

Signs and symptoms of syphilis

Syphilis progresses through four stages:

  1. Primary stage: A single sore or multiple painless sores (Chancre) appear at the site of infection, often unnoticed. These sores heal on their own but require treatment to prevent progression.
  2. Secondary stage: Skin rashes, sores in the mouth, and flu-like symptoms may develop. Other symptoms include swollen lymph glands, fever, sore throat, and muscle aches.
  3. Latent stage: No visible symptoms, but the infection remains in the body and can progress to more severe stages.
  4. Tertiary stage: Occurs years after infection if left untreated, affecting vital organs such as the heart, brain, and nervous system, potentially leading to death.

Syphilis in pregnancy

Syphilis can be passed from a mother to her baby, leading to severe complications such as low birth weight, premature birth, or stillbirth. Babies born with syphilis can face serious health issues like deafness, blindness, or seizures. Regular testing during pregnancy is essential to prevent these complications.

How is syphilis diagnosed?

Syphilis is typically diagnosed through a blood test, though fluid from sores can also be tested in certain cases. Early diagnosis is essential for effective treatment.

What is the Syphilis Antibodies IgG/IgM blood test?

This is a blood test that checks whether your body has made antibodies against the syphilis bacteria. Antibodies are part of your immune system’s way of fighting infections.

There are two main types this test looks for:

  • IgM antibodies – These usually appear early in the infection. If these are positive, it might mean the infection is recent or active.

  • IgG antibodies – These appear a bit later and stay in your body long-term. If these are positive, it might mean you’ve had syphilis in the past or have had it for a while.

So, what does a positive result mean?

  • IgM positive, IgG negative or weak – likely a new infection.

  • IgM and IgG both positive – could mean an ongoing infection. 

  • IgG positive, IgM negative – probably a past infection or you’ve been treated before.

Is there a cure for syphilis?

Can you get syphilis again after treatment?

Even after successful treatment, it is possible to get syphilis again. Follow-up testing is essential to ensure the infection is fully treated, and both you and your sexual partners should be tested and treated to avoid reinfection.
