Why Do Antihistamines Seem to Stop Working?

Many people notice that antihistamines which once controlled their hay fever symptoms no longer feel as effective over time. This can be frustrating, especially during peak pollen season.

In reality, antihistamines do not usually “stop working” completely, but several factors can make them seem less effective:

  • Higher pollen exposure – Symptoms may worsen during intense pollen seasons, overwhelming the effect of standard medication doses.
  • Ongoing nasal inflammation – Antihistamines mainly block histamine, but hay fever also causes broader inflammation inside the nose and sinuses.
  • Incorrect timing – Antihistamines work best when taken regularly before and during pollen exposure, rather than only after symptoms become severe.
  • Nasal congestion dominance – Blocked noses and sinus pressure often respond better to steroid nasal sprays than antihistamines alone.
  • Different allergy triggers – Some people develop additional triggers such as dust mites, mould, or pet allergies alongside seasonal pollen allergy.
  • Poor absorption or inconsistent use – Missing doses or switching products frequently can reduce symptom control.

Sometimes symptoms thought to be “hay fever” may actually be:

  • Chronic sinusitis
  • Migraine
  • Viral illness
  • Non-allergic rhinitis
  • Nasal polyps

At OneMedicine, we help identify why symptoms are persisting and tailor treatment to the individual. This may include combining therapies, optimising nasal spray technique, reviewing triggers, or considering alternative treatments for severe seasonal symptoms.
