Presurgical Optimisation

A regional specialist service

Our Presurgical Cardiac Evaluation Assessment offers a streamlined and expert-led service to evaluate and optimise patients’ health, focusing on cardiovascular health, prior to undergoing surgical procedures. Whether your surgery is minor or major, inpatient or outpatient, we ensure you are medically fit from a cardiac standpoint. Our goal is to minimise risk of complications, enhance surgical outcomes, with rapid restoration of good health.

Why is Presurgical Optimisation Important?

Surgery places stress on the body, especially the heart. For patients with known or hidden cardiac conditions, this can increase the risk of complications such as heart attack, arrhythmias, or unstable blood pressure during or after surgery.

Presurgical cardiac optimisation helps:

  • Identify underlying cardiac risk factors before they become problems

  • Reduce the risk of perioperative complications

  • Ensure you are on the right medications and that any cardiac conditions are well controlled

  • Give surgeons and anaesthetists confidence that you are safe to proceed

  • Improve recovery outcomes and reduce the chances of cancellation or delays

A thorough cardiac review before surgery could be the key to a smoother, safer procedure and faster recovery.

Who is this service for?

In the NHS, this service is reserved for those patients deemed to be at the highest risk of complications during surgery.

However we believe that this service should be available to anyone who needs it.

  • Any patient concerned about their risk of surgery
  • Patients undergoing moderate to high-risk surgery

  • Patients with limited exercise tolerance or symptoms such as chest pain, palpitations, or shortness of breath

  • Patients with a history of heart disease, hypertension, diabetes, or stroke
  • Patients with abnormal ECGs or raised cardiac risk factors on preoperative screening

What is involved ?

  • Detailed Clinical Assessment: Comprehensive history and physical examination focused on cardiovascular risk factors

  • 12-Lead ECG: Performed on-site to assess for arrhythmias, ischemic changes, or conduction abnormalities

  • Echocardiogram (if indicated): Referral or coordination for transthoracic echo to assess cardiac function and valvular health

  • Spirometry: To provide a detailed measure of your lung health. Performed on-site using TrueFlow Technology, and interpreted by a Doctor.
  • Blood Tests: Including BNP, full blood count, renal profile, and lipid panel as needed

  • Risk Stratification: Using established guidelines (e.g., Revised Cardiac Risk Index) and tailored recommendations for surgical planning

  • Medical Optimisation: Adjustment of medications such as beta-blockers, anticoagulants, or antihypertensives where appropriate

  • Communication with Surgical Team: Clear written reports shared with your surgical and anaesthetic teams to facilitate coordinated care

  • Fast Turnaround: Short Notice appointments and reports available for time-sensitive cases
